Please bring all available documentation regarding any injury or prior surgery. If any diagnostic tests have been performed, please bring copies of these tests results with you. In the case of Xrays, MRIs, CT Scans, and bone scans, original films are preferred and can be brought on a compact disc or DVD.Existing Patients
